Role Purpose: The Basic Grade Pharmacist will work as part of a dedicated pharmacy team to ensure delivery of a comprehensive pharmacy service to all areas. The post holder will be responsible to the Head of Department. The post-holder will rotate through Oncology (aseptic preparation), Clinical services, Critical Care and Dispensary. Qualifications/Experience: Post-Graduate qualification in clinical pharmacy or equivalent Be registered in the Register of Pharmaceutical Chemists for Ireland or be entitled to be registered Possess the requisite knowledge and ability for the proper discharge of the duties of the office Have at least 3 - 6 months of hospital experience or equivalent experience Key Responsibilities: Operations Provide pharmacy services and rotate through the dispensary, clinical areas, and aseptic compounding. Prepare, supply and supervise the provision of medicines and other items as may be required. Ensure accurate dispensing and timely distribution of drugs and medicines for inpatients or outpatients. Ensure medication reaches the patient in the correct form and dose. Counselling patients on the effects, dosage and route of administration of their drug treatments, particularly those who require complex drug therapy, when required. Communicating effectively with patients' relatives, community pharmacists, and general practitioners (GPs), etc. Ensuring medicinal products are stored appropriately and securely to ensure highest quality and patients and staffs safety. Supervising and checking the work of pharmacy technicians as necessary Work with the IT Department and clinicians in the development of computerised systems for patient management and ensure that these are maintained and upgraded. Responding to medication-related queries from within the hospital, other hospitals and the general public. Carrying out such duties as outlined in the pharmacy policies. Assist with stock control, ordering and receipting. Participate in the development of retail pharmacy (not currently operational). Ensure all transactions are appropriately recorded on the hospital computer system(s). Assist / deputise for other members of Pharmacy Team as required. Participate in team meetings and committees as required. Learning and Education Contribute to the requirements of professional practice relevant to Pharmacy. Take personal responsibility for own professional development and keep up to date with evidence-based practice and research. Attend and participate in staff development programmes on an ongoing basis. Assist with the orientation and induction of new staff and be available to provide orientation as deemed necessary, helping them to integrate as members of the Unit team. Assist in the training of nursing support staff together. Identify and inform the Learning and Education Team of any training and professional development requirements. Attend all mandatory training days and ensure that all mandatary training is in date. Comply with all HR system and policy requirements. Quality Improvement and Risk Management Ensure compliance with all relevant Blackrock Clinic guidelines, policies, procedures and relevant legislation and regulatory requirements. Participate in the continuous review and evaluation of policies, guidelines and existing practices through regular audit programmes and review engaging with the multi-disciplinary team where required. Assist in promoting a culture of continuous quality improvement across the department. Be aware of the core objectives, standards and key performance indicators for the service and contribute to the monitoring of performance against these standards. Participate in the requirements of the hospitals accreditation process. Participate in the requirements of the hospitals risk management programme. Promote the delivery of a high standard of care to all patients.Encourage evidence-based practice and assist in the dissemination and implementation of research findings. Work with members of the Multidisciplinary team in devising Standard Operating Procedures for the development of the Unit. Assist in ensuring a culture of safety, promoting positive reporting of incidents and near misses; investigate and take remedial action on incidents or near misses involving patients and staff according to hospital policies, protocols and guidelines.
