Key Responsibilities:
Support service users in achieving positive outcomes and greater independence
Deliver person-centred care in line with individual care plans
Promote wellbeing, safety, and social inclusion
Maintain accurate records and reports
Work collaboratively as part of a multidisciplinary team
Requirements:
Qualification in Social Care or related discipline
CORU registration (or have applied and can prove same)
Experience working within disability, residential, or community services desirable
Strong communication and interpersonal skills
Full clean driving licence preferred
