Job Overview:
The Occupational Therapist is a vital member of our multidisciplinary team, responsible for delivering high-quality occupational therapy services to patients across acute settings in Louth.
Key Responsibilities:
1. Conduct comprehensive functional assessments addressing activities of daily living (ADLs), cognition, perception, upper limb function, seating and environmental needs
2. Develop and implement individualised goal-oriented treatment plans in collaboration with patients, families and the multidisciplinary team
3. Contribute to safe and timely discharge planning including home assessments equipment prescription and onward referrals as required
4. Educate patients families carers on functional strategies equipment use rehabilitation goals collaborate with medical nursing physiotherapy speech language therapy social work other relevant disciplines participate audit quality improvement initiatives service development projects
