Unlock Chronic Disease Management Insights
This role involves unlocking the potential value in existing and emerging patient data to support the creation of chronic disease management services.
The Data Analyst will be responsible for extracting, analyzing, and interpreting patient data from Electronic Health Records (EHRs) and screening tools to inform service design, clinical pathways, and strategic partnerships.
This role provides data-driven insights that inform scalable, preventive, and community-based care pathways for patients with chronic conditions such as cardiovascular disease, diabetes, and obesity.
Main Responsibilities:
1. Extract, clean, and analyze relevant patient-level data from internal electronic systems, including EHRs and health screening platforms.
2. Identify patterns and trends in patient demographics, health outcomes, and clinic utilization related to chronic disease.
3. Work closely with clinical leads to co-design datasets and data collection plans aligned with project objectives.
4. Produce timely, visually compelling reports and presentations to inform strategic decisions and service design.
5. Support the development of new, value-based care models aligned with funder and population health priorities.
6. Ensure all work complies with GDPR, data protection policies, and ethical standards.
7. Liaise with operational teams to ensure consistent and high-quality data capture.
8. Contribute to the development of business cases for new or adapted services within the Institute.
9. Monitor and evaluate key indicators of chronic disease and service performance.
10. Participate in project evaluation and service scalability assessments.
About the Role:
This is a 6-month fixed-term position to assess the strategic value of enhanced data analytics within chronic disease services. There may be an opportunity for extension or permanent employment based on outcomes and service growth.
Requirements:
* Bachelor's degree in data analytics, public health, health informatics, computer science, statistics, or related discipline.
* Relevant experience in data analysis, public health intelligence, or applied health research.
* Proficiency in data analysis tools (e.g., Excel, SQL, Python, R, or similar).
* Ability to interpret and present complex data in a clear, accessible way for clinical and non-clinical audiences.
* Strong understanding of GDPR and healthcare data governance principles.
