You'll work closely with a multidisciplinary team to help individuals overcome challenges, break the cycle of homelessness and prepare them for independent living.
Key Responsibilities:
1. Act as a key worker for individual young people, providing support and guidance to achieve their goals.
2. Support individuals who have experienced trauma and challenging behaviors, promoting their physical and emotional well-being.
3. Develop and maintain individual placement plans in line with statutory care plans, ensuring continuity of care.
4. Collaborate with external agencies to secure housing, education, and social support services for individuals.
5. Maintain accurate records in compliance with health and safety standards, ensuring confidentiality and data protection.
6. Participate in a 7-day, 24-hour roster, demonstrating flexibility and adaptability.
Required Qualifications and Skills:
7. Degree in Social Care or a related field, demonstrating a strong foundation in theoretical knowledge and practical skills.
8. Experience in residential care or working with children with complex needs, showcasing ability to handle challenging situations.
9. Knowledge of HIQA standards, Child Protection, and Therapeutic Crisis Intervention, ensuring adherence to best practices.
10. Strong communication skills and a teamwork mindset, facilitating effective collaboration and conflict resolution.
11. Full, clean driver's license, enabling safe transportation and travel.
