Summary of Role:
The purpose of this job is to carry out a wide range of designated social work duties, in accordance with Ability West's policies and procedures as set out in the General Procedures Manual, Operating Instructions, Work Instructions for the Social Work Department and the organisational Safety Statement, and to work as part of the Social Work Team
Duties and Responsibilities:
* To be responsible, subject to the general direction and control of the Head of Social Work, for social work duties associated with the services provided by Ability West.
* To provide a Social Work service to individuals and families who use Ability West services.
* To liaise with team members, Respite and Community Services Manager, Home Sharing Co-ordinator and other relevant Ability West staff.
* To promote service innovation in accordance with best practice.
* To work as part of a number of inter-disciplinary and multi-disciplinary teams to promote the best interests of service users and their families.
* To keep such records and submit such reports, as may be required, by the Head of Social Work.
* Maintain up to date records of all contacts and meetings.
* To perform such other duties, appropriate to the post as may be assigned by the Head of Social Work.
* Candidates must satisfy Department of Health and Code of Professional Conduct and Ethics for appointment by holding a recognised degree or equivalent that is approved by CORU (Social Workers Registration Board).
* Full current driving license with access to your own vehicle.
