Job Summary
The role of a Day Activities Facilitator involves working intensively with individuals to discover what constitutes a good life for them and establishing the necessary supports to achieve their goals. As part of a team in Dun Aoibhinn Day Services, you will work collaboratively with individuals, their families, and other important people in their lives.
About the Role
The person appointed will be responsible for developing and implementing individualized plans to support individuals in achieving their goals. This will involve assessing the individual's needs, identifying resources, and coordinating services to meet those needs.
Responsibilities
• Work in partnership with individuals, their families, and other important people in their lives to develop and implement individualized plans;
• Assess the individual's needs and identify resources to meet those needs;
• Coordinate services to support individuals in achieving their goals;
• Develop and maintain relationships with community agencies and services to ensure access to necessary resources;
