Job Opportunity:
As a member of our multidisciplinary team, you will play a vital role in providing high-quality, person-centred care to individuals with mental health difficulties in the community.

Responsibilities:
* Conduct comprehensive assessments and risk evaluations to ensure optimal patient outcomes.
* Develop individualised care plans in collaboration with patients and the multidisciplinary team.
* Deliver evidence-based interventions, including psychoeducation, medication management, and supportive counselling.
* Act as a keyworker for allocated clients, coordinating their care pathway and providing ongoing support.
* Support patients in managing symptoms, preventing relapse, and engaging with community resources.
* Participate in regular team meetings and contribute to case reviews.
* Promote recovery-oriented and trauma-informed care practices throughout your work.
* Respond to crisis situations and participate in emergency interventions when required.
* Maintain accurate, up-to-date clinical records in accordance with established policies.
* Liaise with primary care, social services, housing, and voluntary agencies as necessary.

Requirements:
* Hold a degree in Social Work and be registered with CORU.
* Possess post-registration experience in mental health, preferably within a community setting.
* Hold a full, clean driving licence and have access to your own vehicle.
