Help people who have mental health conditions live more successful lives. Work as part of a specialist multidisciplinary team to provide a holisticapproach to assist each patient. To understand the unique challenges of those with whom they work andhelp them as necessary. Work collaboratively with team members carrying out Initial Assessments,Individual Sessions and Group Sessions. Analyse their living conditions and look out for causes for concern or areasfor improvement. Compile their findings and analysis into reports for the use of others on theteam. Help patients make sense of their situations, better understandthemselves, constructively challenge them to think and help them setachievable goals to improve their lives.
