Overview
On behalf of our client in public health, we are seeking a Social Care Worker to join their team. This is a community-based role supporting vulnerable people from groups experiencing marginalisation, focusing on people with complex social problems and unmet heath care needs.
Working within local policy framework, best practice and quality standards, as part of a clinically led, multi- disciplinary team, the main duties include; the support, care, welfare, advocacy, assessment and case planning with people experiencing homelessness / international protection applicants / Beneficiaries of temporary protection.
Main Duties
* Working with vulnerable people from groups experiencing marginalisation, focusing on people with complex social problems and unmet heath care needs.
* To participate in the development and implementation of service user Care and Placement Plans and to ensure appropriate recording of same.
* To promote the rights and responsibilities of service users.
* To promote welfare of service users
* To support engagement / attendance at medical and clinical appointments. To liaise with, family, general services and other agencies on behalf of the service user.
* To attend meetings in relation to the care and developments of the service user.
* To implement the practices and principles of effective conflict management
* To attend team meetings and to report to the Manager on matters affecting the delivery of service.
* To be accountable for any money spent during the course of duty.
* To make written and verbal reports to the Manager on a regular basis on the progress / development of service user care plans.
* Reporting and recording any incidents, however minor, that may happen in the course of duty.
* To be familiar with emergency procedures and to know who to contact in an emergency.
* To work as a member of a team.
* To keep abreast of current legislation and current professional care knowledge.
* To implement relevant procedures and policies.
* To undertake ongoing professional training and development.
* To attend regular clinical supervision with external provider.
* To perform any other duties that may be assigned from time to time.
Requirements
* A recognised Social Care qualification.
* CORU registration, or proof of application for CORU registration.
* Previous experience in a similar role.
* Strong administration and ICT skills
* High communication skills and strong written and spoken English.
