Job Description:
We are seeking an experienced Daycare Coordinator Home to oversee the provision of care services for clients living with dementia and other support needs in their own homes.
About the Role:
1. Supervise, coordinate, and support the work of Care Workers.
2. Manage recruitment and ensure comprehensive induction and ongoing training for staff.
3. Conduct performance reviews, manage disciplinary or grievance procedures, and promote consistent application of organisational policies.
4. Schedule and maintain staff rosters based on client needs and service efficiency.
5. Prepare and approve staff pay sheets and travel expenses.
6. Maintain accurate records for staff, clients, and care delivery.
7. Liaise with HR, senior management, and the Home Care Coordinator on staffing and operational issues.
8. Ensure compliance with Service Level Agreements and internal policies.
9. Collate and report service statistics to management and external stakeholders.
10. Apply for and manage grant funding and other income streams to support the service.
11. Assess, identify, and manage client needs, and determine the most suitable care service.
12. Prepare detailed care specifications and review care plans regularly.
13. Conduct risk assessments and address health and safety concerns appropriately.
14. Maintain client confidentiality and promote dignity and quality of life.
Key Responsibilities:
1. Promote positive communication between staff, clients, and family carers.
2. Liaise with GPs, PHNs, and other health professionals to promote and expand the service.
3. Organise events and seminars to raise awareness of care services.
4. Provide support and education to family carers and offer phone-based support as needed.
5. Allocate Care Workers to assist in the day care centre on a temporary basis.
6. Plan and deliver stimulating activities that support clients' emotional and cognitive wellbeing.
Qualifications & Experience:
* Experience in care coordination, preferably within dementia or community care services.
* Strong leadership and communication skills.
* Knowledge of care planning, risk assessment, and client support documentation.
* Proficiency in managing teams, rostering, and record keeping.
* Familiarity with health service frameworks and quality standards.
