Consultant Neurologist Job Specification & Terms and Conditions Location of Post This is a Public Only Consultants Contract 2023. The role is 0.5 WTE Mater University Hospital (18.5 hours) and 0.5 WTE the Rotunda Hospital (18.5 hours). Post-holder will participate in the neurology on-call rota in the Mater Hospital on a pro-rata basis. Details of Service Population to be served is the HSE Dublin North East region, with the post to be based in the Rotunda Hospital providing specialist expert input, support and management to WWE and women with other neurological conditions attending the Rotunda for their maternity care and providing consultation services to maternity services located the north east. Working closely with obstetric colleagues in the Rotunda, the post-holder will accept high risk referrals from maternity services located in the north east. In addition to the provision of care to women with neurological conditions related to pregnancy, the post-holder will also contribute to the running and delivery of general neurology and on-call stroke care in the regional neurology hub located in the Mater Hospital. Reporting Relationship To the Master through the Clinical Director. To the Clinical Director of the Mater Hospital for clinical matters relating to the regional neurology hub. Purpose of the Post The HSEs National Women and Infants Health Programme working with the National Epilepsy Clinical Care Programme has identified the absolute requirement for the implementation of an integrated model of care for the management of women with epilepsy (WWE) attending maternity services. A practice guide for the safe management of WWE attending maternity services was developed in 2018 under the governance of the National Epilepsy Clinical Care Programme. However to date the dedicated resources to safely deliver this care in the Dublin North East Region have not been readily available. Neurology services including the neurology regional hub in the Mater are under significant pressure from the overall demands on their specialist service, resulting in tertiary maternity services like the Rotunda Hospital struggling to access structured dedicated specialist medical resources and inputs that are aligned and protected for women attending their services. This presents a very real and significant clinical risk in the management and care of WWE attending maternity services, with this risk being further heightened in circumstances whereby the patient is no longer under the active care or clinical governance of a designated neurologist or neurology service. The need for a resourced and structured integrated model of care between specialist neurology/epilepsy services and tertiary maternity / womens health services is further compounded when other key elements and issues are factored in including the pathway that women travel with epilepsy and how their life decisions regarding their reproductive and sexual health impact the management and their experience of epilepsy, the requirement of the HSE to respond in a robust and structured manner to requirements surrounding the safe prescribing and use of sodium valproate; the specific recommendations of the practice guide regarding WWE attending maternity services and the need for these recommendations to be implemented by the HSE so to enhance the safety of women attending maternity services; and the clinical care needs of women attending maternity and other womens health services who present with neurological conditions new or existing other than epilepsy. Principal Duties and Responsibilities General To carry out the duties of a Consultant Neurologist in line with the POCC23 Consultant Contract; To have a lead regional role in the development and delivery neurology services in the area of maternal health service in the HSE Dublin North East Region. To support and enable the delivery of evidence based information to patients on the full range of issue applicable to their health and well-being. To participate in the teaching of medical and midwifery/nursing staff, including undergraduates and postgraduates across the Rotunda Hospital and the Mater Hospital. To increase awareness and education to other health care practitioners on neurological womens clinical health issues particularly in the maternity health care setting. To be actively involved in the clinical governance aspects of respective departments in both the Rotunda Hospital and the Mater Hospital including the management of risk. To participate in multidisciplinary team meetings with relevant specialists across both sites. To work with the National Epilepsy Clinical Care Programme and the National Women and Infants Health Programme so as to enable and support the implementation of the HSEs Integrated Epilepsy & Maternity Model of Care participating as required in relevant national fora and professional networks. To ensure that written protocols and clinical guidelines are in place in both services as required and that these include and address recommended national and international guidelines and best practice. To support the implementation of HPRA VPA guidance and Pregnancy Awareness / PREVENT Programme. To provide structured and clinical supervision and mentorship to appointed ANP resources in the area of epilepsy care. Utilisation and support for e-health options e.g. use of MN-CMS to monitor the care of women. To support the Irish Epilepsy Pregnancy Register and the collation and review of key data and metrics. Any other duties as deemed appropriate. Rotunda Hospital To align and co-ordinate the care of WWE attending maternity services in HSE Dublin North East. To support the provision of a consistent approach across REOs regarding the care of WWE attending maternity services and that relevant information and data is shared as appropriate at national level. To be responsible for the organisation and provision of specialist neurology services to women with epilepsy and other neurological conditions attending the maternity services in the Rotunda Hospital working closely and collaboratively with consultant obstetric colleagues. Tosupport the existing Rotunda WWE obstetric clinic in collaboration with obstetric colleagues and the advanced nurse practitioner. To provide consultation services by means of telephone consult to maternity services located in the northeast with specific reference to OLOL. To provide inpatient neurology consultation service. To collaborate with Irish Medicines in Pregnancy Service at the Rotunda with regards to optimal provision of tailored antiseizure medicines information to patients. To ensure timely communication with neurology services during pregnancy for women attending WWE obstetric clinic. To be responsible for management of therapeutic drug monitoring results at the Rotunda hospital. Mater Hospital To attend and support the longitudinal care epilepsy clinic. To establish and provide a first seizure clinic. To provide inpatient general neurology in-patient and consultation services. To participate in the existing hospital neurology service including on-call care in a pro-rata manner. Eligibility Criteria Qualifications and/ or experience Professional Qualifications Registration as a specialist in the Specialist Division of the Register of Medical Practitioners maintained by the Medical Council in Ireland in the specialty of Neurology. One years postgraduate experience in the area of maternal health (desirable). Demonstrated higher postgraduate degree such as MD or PhD (desirable).
