Main Responsibilities
1. To facilitate the Person Centred Planning process for service users.
2. To identify individual goals and required supports, maintaining accurate records as necessary.
3. To develop and deliver person centred programmes meeting service user needs through Person Centred Planning, maintaining relevant records.
4. To assist individuals in accessing their community by facilitating activities and programmes, and other reasonable duties as deemed appropriate.
Essential Requirements
* A full driving licence is required due to travel involved in this role.
* A minimum QQI Level 5 in Social Care or equivalent qualification is essential.
* A third level graduate in Health & Social Care or equivalent is desirable.
* Programme facilitation skills and client handling experience are advantageous.
