Services are provided at the level of the individual, family and the broader community. In addition to delivering personalised care to individuals, account is taken of the collective health and well-being, the social determinants of health and the nursing care needs of the whole population. Activities therefore include clinical nursing, health promotion and public health and well-being initiatives.The Public Healh Nursing Service works collaboratively with nursing colleagues in the acute, secondary, and end-of-life Palliative Care services and with General Practitioners and Practice Nurses within Primary Care Teams.Under the leadership of the Director of Public Health Nursing, the Assistant Director of Public Health Nursing (ADPHN) will be responsible for the management of the community nursing service delegated to them. They must manage the delivery of an efficient and effective community service to those eligible who require it.
