Delivering a Housing First approach to individuals experiencing long term homelessness requires a team of skilled professionals who can provide a safe and welcoming environment for clients.
The Role:
* Working as part of the housing team to deliver a Housing First approach to individuals experiencing long term homelessness in County Clare.
* Full-time, 1 year fixed term contract with a competitive salary range and annual leave entitlement.

Key Responsibilities Include:
* Providing basic physical care and support within the service, respecting consumer choice and self-determination.
* Establishing trusting relationships with clients who are living on the streets or in temporary accommodation and engaging them in work that enables them to move to a more stable lifestyle.
* Undertaking assessments and support planning with clients as the lead agent using the Care and Case Management system and protocols.
* Developing methods, structures and relationships to support clients with their individual housing, welfare rights and entitlements.
* Assisting tenants to create and sustain a home, implementing practices in line with tenancy settlement indicators.
* Maintaining a portfolio of accommodation providers in social housing, homeless sector and private rented accommodation.
* Accompanying and providing support to clients on appointments to other services.
Requirements/Experience Needed:
* Degree in social care, youth work or related discipline.
* Knowledge and understanding of homelessness and issues that affect people who are or have been homeless.
* Ability to plan and implement programmes.
* Report writing abilities and IT skills.
* Flexibility and communication skills.
