Facilitator Disability (FACCH1803)
Location: Celbridge, Autism A Chara
Contract: Permanent Part-time – 32 hours per week
Key Responsibilities:
Provide person‑centred support to individuals with complex needs.
Plan and implement programmes and person‑centred plans.
Work independently, demonstrating initiative.
Maintain effective communication in fluent English.
Qualifications:
Full QQI level 5 Health Care Qualification (e.g., health, community, care sector).
Full, clean driver’s licence.
Passion for providing person‑centred support.
Other Requirements:
Awareness of the Key Worker/Support Worker role and philosophy.
Strong communication skills.
Employment Details: Community Access Facilitator 6425 – Section 39, Patient and Client Care category.
This role is open to both HSE and non‑HSE applicants.
