Key Responsibilities:
* To promote independent living and community integration by designing programmes tailored to meet service users' needs and capabilities.
* Facilitating educational developmental recreational occupational programmes as appropriate in collaboration with programme teams .
* Maintaining accurate records regarding person-centred planning sessions carried out across both groups or individually
