Job Title: Team Lead/Senior Care Coordinator
The primary goal of this role is to provide high-quality care and support through a person-centred approach, meeting the practical, physical and emotional needs of service users with intellectual disabilities.
To achieve this objective, the Team Lead will be responsible for coordinating shift activities, supervising staff under their direction and working collaboratively as part of a multidisciplinary team in service delivery. They will report directly to the Person In-Charge and may be required to deputise for them in their absence.
Key Responsibilities:
* Provide individualised support plans that promote independence and skill development;
* Supervise staff members on duty and ensure they are following established procedures;
* Assist persons with disabilities in daily living activities such as personal care, meal preparation etc.;
* Collaborate with healthcare professionals to develop comprehensive care plans that meet individual needs.
