Job Description
We are seeking a highly skilled Social Worker to join our team in Dublin. The successful candidate will work with clients and families to provide comprehensive assessment services, formulate safeguarding plans and care plans, and assist in the implementation of current legislation.
* Ambulatory Care Services: Providing home visits, supporting families and caregivers on management strategies, psychoeducation, liaison with respite or day services.
Key Responsibilities:
1. Provide clinical case management for caseloads of service users;
2. Maintain accurate records of client interactions;
3. Liaise with multidisciplinary teams including mental health staff for collaborative support services;
4. Demonstrate strong professional boundaries while upholding core values such as empathy & compassion when interacting w/ patients 'families etc.”,
