Inclusion Health is evolving into an established approach to healthcare in Ireland, both in hospital and community settings, that focuses on understanding and meeting the health needs of individuals who experience social exclusion. Individuals who experience one or more aspects of social exclusion, such as homelessness, drug addiction, incarceration, or belonging to a minoritised ethnic group, face high levels of ill health and significant challenges in accessing healthcare. This results in poor management of chronic medical conditions with subsequent morbidity, mortality, and preventable ED attendances or hospital admissions.
Early identification and engagement with this cohort of patients in the Emergency Department (ED) will identify those who may be effectively managed through ongoing outpatient/community services, outreach programmes, or other pathways within the hospital.
The RANP/cANP will employ advanced clinical judgment and assessment skills to perform a clinical assessment of patients and will initiate timely treatment plans, thereby accelerating the decision-making process and determining the most appropriate care plan for each individual.
Additionally, the RANP/cANP will collaborate closely with interdisciplinary teams, including ED, hospital, and community-based Inclusion Health services, clinical teams, nursing staff, and allied health professionals, to ensure seamless and timely patient transitions between services. This collaboration will support the development of care pathways that are responsive to the complex needs of this cohort of patients and also align with best practices in emergency care.
By focusing on effective patient management and strategic decision-making, the RANP/cANP will play a crucial role in enhancing satisfaction and promoting a high standard of patient-centred care to this vulnerable patient cohort within the ED. This innovative approach aligns with our commitment to delivering efficient, patient-centred care while fostering a culture of collaboration and excellence within the healthcare team.
