Job Title: Grade CNM2 Health and Chronic Disease Management for Adults with an Intellectual Disability
Remuneration: The salary scale for the post is:
Duration of Post: If the post is awarded as CNM2 in Health and Chronic Disease Management, the successful candidate is required to progress to Clinical Nurse Specialist Grade within 2 years of commencement of this post and formally apply for entry onto the ONMSD CNS database.
Location of Post/Organisational Area: The CNS/CNM2 operates within St John of God Community Services North East Residential Service, providing residential care across Counties Louth, Meath, and Monaghan, supporting adults with intellectual disabilities. The role involves clinical nursing care, advice, and leadership across these locations.
The position is strategically placed to:
* Provide clinical leadership across multiple care settings
* Develop and implement service improvements
* Lead clinical governance initiatives
* Foster relationships with primary and acute care services
The service offers specialised residential and day services, delivering expert clinical care, advice, and leadership to adults with intellectual disabilities, addressing complex health needs and promoting wellbeing.
The team includes various managerial, clinical, social care, and multidisciplinary staff, including physiotherapists, occupational therapists, speech therapists, and social workers.
Future developments include enhancing person-centered care models, developing evidence-based guidelines, staff training, and quality initiatives.
Reporting is to the Nursing Practice Development Coordinator and Director of Nursing Care and Support. The role involves collaboration with internal and external stakeholders, including families, healthcare providers, education institutions, and statutory bodies.
The purpose of the post is to deliver care aligned with the Framework for the Establishment of Clinical Nurse/Midwife Specialist Posts, focusing initially on residents of the service.
Core concepts include Clinical Focus, Individual Advocacy, Education and Training, Audit and Research, and Consultancy, with duties spanning direct and indirect patient care, advocacy, education, research, leadership, and service development.
Key responsibilities involve comprehensive assessment, care planning, monitoring, collaboration, medication management, health promotion, advocacy, education, research, quality improvement, leadership, safety, and management.
Minimum requirements include registration with the Nursing and Midwifery Board of Ireland, relevant experience, postgraduate qualification at Level 9 or higher, CPD, and ability to practice safely. Candidates must demonstrate knowledge of legislation, standards, clinical governance, and possess IT and communication skills. They must also have a full driver’s license and be willing to travel.
