We are currently recruiting a Clinical Nurse Specialist – Inflammatory Bowel Disease (IBD) for a hospital in Dublin, Ireland .
Responsibilities Provide a specialist IBD nursing service to patients presenting to BSD, delivering evidence-based care and support across the full continuum of care.
Undertake comprehensive holistic patient assessments, incorporating physical, psychological, social, and spiritual needs, and develop, implement, and review individualized care plans in collaboration with the MDT, physicians, patients, and families/carers.
Monitor, evaluate, and modify care plans and treatment responses in line with agreed clinical pathways, policies, procedures, protocols, and guidelines (PPPGs).
Accept and manage appropriate referrals, coordinating investigations, treatment therapies, medication reconciliation, and follow-up care, with a strong focus on polypharmacy and medication safety.
Communicate effectively with patients and families/carers to assess needs and provide education, counselling, advice, and ongoing support to promote self-management and health promotion.
Work collaboratively with multidisciplinary colleagues across primary and secondary care to ensure seamless, patient-centred service delivery.
Deliver specialist nursing advice and interventions for patients with IBD, recommending and implementing changes to care plans within scope of practice based on current clinical evidence.
Maintain accurate clinical documentation, monitor treatment programmes, evaluate outcomes, and prepare reports on effectiveness and future therapeutic requirements.
Practice in accordance with the Scope of Nursing and Midwifery Practice, relevant legislation, and the Codes of Professional Conduct and Ethics of the Nursing and Midwifery Board of Ireland.
Participate in MDT meetings and case conferences, support safe medication management practices, advise patients and carers on medication safety, and submit required reports, including monthly activity reports to the Assistant Director of Nursing and BSHS.
Requirements Current registration with the Nursing and Midwifery Board of Ireland (NMBI).
Successful completion of a post-registration programme of study relevant to the specialist field of Inflammatory Bowel Disease.
A minimum of one year's experience working in the field of Inflammatory Bowel Disease, with evidence of ongoing Continuing Professional Development (CPD) relevant to the specialist area.
Demonstrated specialist knowledge and ability, including a high standard of professional suitability and the clinical, leadership, managerial, and administrative competence required for the effective discharge of the duties of the role.
