The Assessment Specialist will report to the Business Manager Referrals and Contracts

The Assessment Specialist will play a key role in informing placement decisions across Resilience Healthcare services. Using strong clinical and social care assessment skills, the post holder will review referral information, complete person-centred assessments in a variety of community settings, and produce high-quality assessment reports that guide decisions made by the Referral Committee.

Assessment & Clinical Review
Review referral files to determine whether individuals may be suitable for Resilience Healthcare Services.

Conduct comprehensive, person-centred assessments in community settings across Ireland.

Use professional assessment skills to observe how an individuals disability impacts their daily life and functional abilities.

Engage directly with individuals, families, carers, and professionals to gather additional information.

Produce clear, accurate, and evidence-based assessment reports to support decision-making by the Referral Committee.

Recommend the service model or supports that best meet the individuals needs, aligned with Resiliences care pathways.

Client & Stakeholder Engagement
Build rapport and trust with individuals and families during the assessment process.

Communicate professionally and empathetically with MDT members, HSE Disability Teams, advocates, and other stakeholders.

Represent Resilience Healthcare positively and professionally at all assessment visits and meetings.

Operational & Administrative Responsibilities
Maintain accurate, confidential records in line with GDPR and safeguarding obligations.

Update and manage assessment information on Dynamics 365 and other internal systems.

Follow all Resilience Healthcare policies, procedures, and regulatory standards relevant to assessment and pre-admissions.

Collaborate with the scheduling team, who will arrange assessment visits, ensuring punctuality, preparedness, and professionalism.

Service Knowledge & Continuous Learning
Develop a strong understanding of Resilience Healthcares service models, including Residential, Supported Living, Shared Care, and Respite Services.

Conduct familiarisation visits to Resilience services to maintain up-to-date operational and environmental knowledge.

Attend required training sessions, CPD opportunities, and reflective practice meetings to enhance professional skill and knowledge.

Contribute insights that support continuous improvement of assessment tools, templates, and processes.

Travel & Mobility
Travel throughout Ireland to complete assessments, using a company vehicle provided for the role.

Ensures assessments, reports, and records are completed to the highest standard.

Essential/Role Requirements
3+ years of experience in disability services, social care, behavioural support, clinical roles, or assessment-based work.

A recognised qualification in Social Care, Psychology, Applied Behaviour Analysis, Nursing, Occupational Therapy or a related field.

Demonstrated experience conducting functional, clinical, or holistic needs assessments.

Strong report-writing skills and attention to detail.

Ability to work independently while being part of a supportive and collaborative team.

Highly organised, with strong time-management skills for on-the-road work.

Strong IT literacy; experience with Microsoft Dynamics is desirable but not essential.

Full clean driving licence and willingness to travel nationwide.
