We're looking for a Care Center Social Worker who shares this vision.
Comfort.
Bring a sense of calm and comfort while giving support and counseling to patients and their families, as well as assisting with crisis intervention when needed.
Play a key role in the development of treatment for every patient and family, serving as both a consultant and team member who is actively involved in providing direct services.
Advocate.
Utilize existing community resources to better accommodate the needs of patients and their families, including referring them for financial assistance if there's a need.
Act as a liaison between social agencies and the Angela Hospice team.
Be there for families and patients if they wish to develop an advance directive.
If need be, provide families with guidance for funeral planning.
Connect.
Complete patient admission procedures, such as initial biopsychosocial assessment, and creating a care plan.
Keep patients' records and documentation up-to-date.
Work with members of the Interdisciplinary Group (IDG) to help them better understand the significant social and emotional factors related to patient care.
Act as a resource for hospice team members for services available in the community that can be used to meet the psychosocial/economic needs of patients.
What You Bring
A graduate of a Master's program in social work accredited by the Council on Social Work Education (CSWE)
Current licensure in the State of Michigan, LMSW or LLMSW
Minimum of one (1) year's experience in health care, hospice experience preferred
Understanding of the hospice philosophy, and issues of death/dying
Good verbal/written communication, and organization skills
