The candidate ANP Diabetes Integrated Care is primarily focused on promoting new ways of working collaboratively to improve the quality of care delivered by those who work within the whole care system (DOH The Candidate ANP (Diabetes Integrated Care) works within Chronic Disease Hub/Primary Care setting in Cavan/Monaghan. The Candidate ANP (Diabetes Integrated Care) practices in collaboration with the Consultant (Diabetes Integrated Care) and Diabetes Integrated Multidisciplinary Team.The objective is to promote safe, quality, person-centred care delivery for patients, providing timely and safe access to care. The Candidate ANP will work in collaboration with the Diabetes team within the geographical catchment area of Cavan Monaghan, supporting an integrated approach to the provision of care to patients with Diabetes conditions, through the Integrated Model of Care for people with Type 2 Diabetes Mellitus and NCEC National Clinical Guideline for Type 1 Diabetes in Adults Once registered as an RANP, the caseload and criteria for referral to the RANP, (Diabetes Integrated Care) will be agreed in consultation with the Consultant(s) in Diabetes Integrated Care. In line with Sláintcare's objectives, the service aims to deliver more integrated care, supporting care nearer the home, helping to reduce and prevent hospital visits and supporting the ultimate goal of reducing waiting lists and reducing waiting times.The Candidate ANP will contribute to, in collaboration with the integrated multidisciplinary team, collect KPI data and report quarterly and annual reports on activity levels and key outcomes achieved within the service using this activity data to build service plans for following years.The Integrated Diabetes/Chronic Disease Service Model sets out the end to end service architecture for the identification and management of people living with chronic disease. The focus is on providing an end-to-end pathway that will reduce admissions to acute hospitals by providing access to diagnostics and specialist services in the ambulatory care hubs in a timely manner. For patients who require hospital admission, the emphasis is on minimising the hospital length of stay, with the provision of post-discharge follow up and support for people in the community and in their own homes, where required. A shared local governance structure across the IHA will ensure the development of a fully integrated service and end-to-end pathway. The Candidate ANP (Diabetes) will work as a member of the integrated care team for Diabetes in the Chronic Disease Hub, identifying an appropriate care pathway for clients and informing their plan of care. The HSE service plan and all recent strategies and policies aim to avoid hospital admission and plan care in an appropriate setting when necessary. The role will support the establishment of integrated care pathways for Diabetes.The Model of Care outlines a continuum of services from self-management to care in the community, the specialist ambulatory care hub for chronic disease and care in the acute setting.Essential CriteriaEligibility CriteriaQualifications and/or ExperienceCandidates must have at the latest date of application:Statutory Registration, Professional Qualifications, Experience, etc.(a) Eligible applicants will be those who on the closing date for the competition are :1. Be currently undertaking a Master's Degree in Nursing (Advanced Practice Pathway) or be eligible to register to undertake additional Level 9 National Framework ofQualifications (Quality and Qualifications Ireland) specific modules of a Master's Degree in Nursing (Advanced Practice Pathway) within an agreed timeframe.Educational preparation must include at least three modular components pertaining to the relevant area of advanced practice, in addition to clinical practicum Orv.
