Role Overview: A Client Care Coordinator ensures clients receive high-quality care by coordinating services, managing schedules, and communicating effectively with clients, care teams, and healthcare professionals. They also maintain accurate records, handle inquiries, and assist with administrative tasks.
Key Responsibilities:
* Scheduling and Coordination: Managing client care schedules, ensuring appropriate coverage, and adjusting schedules as needed.
* Communication: Acting as a liaison between clients, their families, care team members, and healthcare professionals.
* Client Support: Addressing client inquiries, concerns, and requests, providing support and guidance.
* Record Keeping: Maintaining accurate and up-to-date client records, documentation, and care plans.
* Quality Assurance: Monitoring client care delivery to ensure high standards of care.
* Administrative Tasks: Assisting with data entry, filing, report generation, and other administrative duties.
Skills and Experience:
* Communication and Interpersonal Skills: Ability to communicate effectively with diverse individuals, both verbally and in writing.
* Organisational Skills: Managing multiple tasks, prioritising workload, and maintaining accurate records.
* Problem-Solving: Assessing situations, identifying problems, and implementing effective solutions.
