About the Role
The Social Care Worker plays a key supportive and advocative role for individuals supported by our service.
The primary focus is on supporting the person in all aspects of their life, promoting independence and development of skills.
This involves assisting with practical tasks associated with daily life, either in the person's home or within a day service setting.
The Social Care Worker will also assist in developing individualised person-centred plans and achieving associated outcomes.
They will support people in making informed choices and realising their full potential.

Key Responsibilities
* Supporting individuals in all aspects of their life, focusing on promoting independence and skill development.
