Clinical Nurse Managers play a pivotal role in ensuring the safe and effective management of patient referrals to hospitals from acute settings. They facilitate admission and discharge planning, acting as case managers for patients as required.
Key Responsibilities:
* Receive patient referrals from acute hospitals; review referrals and liaise with Discharge Coordinators
* Ensure timely discharges by implementing effective communication with referral sources and discharge agencies
* Arrange for accepted referrals to be admitted to units
* Act as case manager for patients as required and as agreed by Patient Flow
* Maintain accurate and informative assessments and records within hospitals in accordance with legislation and standards
Requirements:
To succeed in this role, candidates must possess 3 years post-registration experience in relevant fields along with a recognised Managerial Qualification. Additionally, they must be registered on the NMBI live register and have clinical experience working in older persons care or nursing home care. Strong communication and listening skills are essential for effective collaboration with multidisciplinary teams.
