About the Role:
The successful candidate will play a key role in conducting thorough assessments, developing tailored rehabilitation programs, and implementing evidence-based interventions for older adults recovering from illness, injury, or surgery.
Key Responsibilities:
1. Conduct comprehensive evaluations of patients' physical, cognitive, and psychosocial needs.
2. Develop and implement individualized treatment plans focused on maximizing functional independence.
3. Work collaboratively with multidisciplinary teams, including physiotherapists, nurses, and medical staff.
4. Lead discharge planning efforts to ensure seamless transitions back to home or community settings.
5. Provide expert advice on manual handling and equipment recommendations to enhance patient mobility and safety.
6. Supervise and mentor junior occupational therapists and assistants.
7. Maintain accurate and detailed clinical records in line with best practice standards.
8. Educate patients, families, and caregivers on effective rehabilitation techniques and adaptive strategies.
