Case Manager, Settlement Services, Dublin

This role provides visiting support to households moving out of homelessness, helping them settle into their new homes and communities.
Working in partnership with Dublin City Council, you'll empower customers to build the skills and confidence needed to manage their tenancy and maintain housing stability.
What you will do:
Work within Focus Ireland's overall objectives and comply with the Service Level Agreement with Dublin City Council.
Ensure adherence to Dublin City Council's Generic Housing and Health & Safety Policies.
Manage and maintain the agreed caseload effectively.
Engage customers in line with Dublin City Council Tenancy Agreements, including rent collection, maintenance, and estate management responsibilities.
Contribute to needs assessments and support plans using the PASS system, and keep accurate casework records.
Provide one-to-one visiting support to help customers make informed decisions and comply with tenancy responsibilities.
Collaborate with the Manager/Team Leader to design and implement support packages that promote household welfare.
Work as part of a multi-disciplinary and multi-agency team, maintaining links with Gardaí, Probation, HSE, medical services, community services, and Local Authorities.
Ensure effective communication, referrals, case reviews, discharge planning, and case conferencing with internal and external stakeholders.
Attend meetings, supervision sessions, and training as required; implement all policies and procedures, including Health & Safety, Child Protection, and Working Alone policies, and remain vigilant to workplace risks.
What you will need:
BA in Social Care or equivalent qualification.
2 years experience of working with families and individuals experiencing homelessness.
Experience of lone working in a social care setting.
Experience of working and liaising with multiple agencies.
Computer Literacy.
Report writing skills.
Full clean driver's licence (Desirable)
