Responsibilities
* Be part of a specialist multidisciplinary team to provide a holistic approach to assist each patient.
* Help people who have mental health conditions live more successful lives.
* Understand the unique challenges of those with whom you work and provide appropriate support.
* Work collaboratively with team members carrying out Initial Assessments, Individual Sessions and Group Sessions.
* Analyse living conditions and identify causes for concern or areas for improvement.
* Compile findings and analysis into reports for use by the team.
* Help patients make sense of their situations, set achievable goals, and support them to improve their lives.
* Work within legal frameworks (e.g., Mental Health Act 2001) and undertake continuing professional development as appropriate.
* Adhere to all company policies and procedures and use company IT systems responsibly and securely.
* Demonstrate reliability, flexibility and the ability to work to deadlines under pressure.
Qualifications & Experience
* Have a recognised qualification as a Social Worker with CORU registration.
* Minimum of 3 years experience within a similar social care setting.
* Effective verbal and written communication and IT skills; good problem-solving and decision-making abilities.

* Effective verbal and written communication; strong IT literacy.
* Ability to learn quickly and work independently as well as in a team.
* Empathy, reliability, flexibility and self-motivation.
* Ability to manage deadlines and work under pressure.
