The Homecare Project is now seeking a dedicated and compassionate individual to join our team as a Project Support Specialist. This role will provide essential support to tenants under our programme, empowering them to achieve their housing and recovery goals.
Job Description:
As a Project Support Specialist, you will work within the framework of our overall objectives and policies, ensuring the best possible outcomes for our service users. Your key responsibilities will include:
* Conducting needs assessments and developing agreed care plans with service users
* Supporting service users in accessing recovery-focused training, education, and employment opportunities
* Collaborating with multi-disciplinary teams and services to ensure seamless support
* Maintaining accurate casework records, daily logs, and written assessments
* Working closely with homecare support workers to coordinate and implement care plans
Required Skills and Qualifications:
To be successful in this role, you will require:
* 2-year post-qualification experience in a related field
* Level 7 or above in social care
* Full clean driving license and access to a car
* Excellent communication, problem-solving, and interpersonal skills
