Job Description:
1. A dynamic and flexible support worker is required to work in our community-based service.
2. The role will involve supporting individuals in their own homes, promoting independence and autonomy through person-centred approaches and interventions.
3. This includes providing emotional support and practical assistance with daily tasks such as cooking, cleaning, shopping etc. as well as assisting the individual to develop life skills which may foster greater levels of self-reliance.
Supporting the individual to maintain a safe and comfortable living environment including tasks such as meal preparation planning,,
enhancing routines for tenancy sustainment ensuring the wellbeing of the tenant always remains at forefront. A further core component is linking them into opportunities around leisure activities classes workshops so they can enhance their knowledge on various aspects ,reducing isolation increasing social interaction ultimately aiding long term mental health progress – this comprehensive role shall ensure stable housing safety care needs met effectively guiding successful pathway forward leading towards hopeful future.,
