Main Responsibilities
1. Facilitate the Person Centred Planning process to support service users effectively.
2. Determine individual goals and required supports, maintaining accurate records as needed.
3. Develop and implement person centred programmes tailored to meet service user needs, documented in required records.
4. Assist individuals and groups in accessing community resources to address identified needs through facilitated activities and programmes.
The successful candidate will report directly to the Community Service Manager via the Programmes Supervisor.
Required Skills and Qualifications
* Minimum QQI Level 5 in Social Care or equivalent qualification.
* A third level graduate in Health & Social Care or equivalent is preferred.
* Full Manual Driving Licence Required.
* Experience supporting adults with intellectual disabilities and mental health issues.
* Programme facilitation skills are advantageous.
* Client handling experience is beneficial.
