The Role:
* You will be working closely with clients to understand their needs and develop individualised plans to support them into stable accommodation.
* This is a full-time position, 1 year fixed term contract with a competitive salary range and attractive benefits tailored to employee needs.

Key Responsibilities:
* You will provide a safe and welcoming environment for clients that are out of home or at risk, respecting consumer choice and self-determination.
* You will establish trusting relationships with clients who are living on the streets or in temporary accommodation and engage them in work that enables them to move to a more stable lifestyle.
* You will undertake assessments and support planning with clients using the Care and Case Management system and protocols.
* You will develop methods, structures and relationships to support the client with their individual housing, welfare rights and entitlements.
* You will assist the tenant to create and sustain a home, implementing practices in line with tenancy settlement indicators.
Requirements/Experience Needed:
* Degree in social care, youth work or related discipline.
* Knowledge and understanding of homelessness and issues that affect people who are or have been homeless.
* Ability to plan and implement programmes, report writing abilities and IT skills.
* Flexibility and communication skills, full driver's license with access to car desirable.
