Job Description
We are seeking an experienced Care and Support Team Manager to join our team. As a key member of our team, you will be responsible for planning, coordinating and implementing safe transitions of new individuals into our service and community.
Key Responsibilities:
* Plan, coordinate and implement safe transitions of new individuals into our service and community.
* Liaise with families and multi-disciplinary teams to promote individual needs.
* Develop and implement individual support programmes to enhance capabilities and independence.
* Coordinate ongoing assessment and review through completion of suitable assessment models.
* Support physical, emotional and personal needs of individuals, upholding principles of respect, privacy and dignity.
* Ensure medication is stored and administered according to policy and procedures.
* Provide effective leadership and supervision to support workers, including time and attendance monitoring.
* Maintain accurate records and reports in line with organizational policy and statutory requirements.
Requirements
To be successful in this role, you will need:
* 2 years' experience in a health and social care setting and minimum of QQI Level 5 in Health Care or equivalent.
* Full valid driving licence and access to own vehicle (if disability precludes holding driver's licence, state how mobility requirement will be met).
* Strong verbal and written communication skills, report writing skills and computer skills.
* Risk assessment, leadership and problem-solving skills, organisational and time management skills.
* Ability to work flexibly and availability to work night shifts when required.
