Job Title: Senior Care Manager

* Leading and managing a team of staff, including supervision, appraisals, and training.
* Coordinating and prioritising appointments with the multi-disciplinary team.
* Ensuring high-quality person-centred care plans, health promotion, reviews, and personal risk assessments are implemented.

About the Role
