Overview
Support Coordinators play a critical role in facilitating practical support and coordinating services for older people to address their challenges effectively. The role is field based and requires travel within the community. The position is Full time – Permanent, located in South East Wexford. Drivers license and own car are essential.

Responsibilities
* Support Coordination: devise personalised support plans to address challenges and enhance the older person’s quality of life; facilitate access to ALONE’s services and help older people access other services in the community.
* Partnership Engagement: establish and nurture referral relationships with statutory and non-statutory agencies in the designated area to broaden access to support services; develop and maintain efficient referral pathways for ALONE’s Support Coordination services.
* Performance Management and Reporting: adhere to ALONE’s quality management systems and policies; monitor and manage KPIs to maximise reach and impact; deliver thorough reports on service developments, challenges and achievements; manage the distribution and installation of technology that enhances wellbeing and safety.
* Data Management: maintain records on ALONE’s CRM database to inform service delivery and future strategies.
Essential skills & experience
* Demonstrated passion for improving the lives of older people and commitment to ALONE’s mission.
* Problem-solving skills with focus on needs assessment and resource allocation.
* Excellent written and verbal communication, fostering partnerships and negotiating effectively with diverse stakeholders.
* Proficiency in using technology and data management systems to enhance service efficiency and reporting accuracy.
* Good organisational and multitasking abilities with a solution-focused approach to complex challenges.
* Keen attention to detail for accurate record-keeping and effective service monitoring.
* Enthusiasm for setting and achieving SMART targets that maximise positive impact on older people.
* Self-starter with the ability to work well with stakeholders.
* Must have full clean driving licence and own transport.
A background in older people-related fields is not required. The key factor is alignment with ALONE’s mission and the ability to coordinate services and address challenges faced by older people effectively.
Salary & Benefits
* Salary: €37,000 per year
* 35 Hour week (core hours 9-5) – Full-time employees
