Homecare Project Worker

The Homecare Project is a customized model of high-level support designed to promote independent living in the community for mental health service users whose tenancies or home occupation are at risk due to ongoing challenges with self-neglect, hoarding, and/or incapacity to maintain their home to an adequate standard.

This collaboration between Carlow County Council, the Carlow/Kilkenny HSE Mental Health Services, and the Good Shepherd Centre aims to provide secure tenancies with support as a way forward for individuals with complex needs.

* Work within the framework of overall objectives and policies of our organization.
* Acknowledge and support service user preferences for fuller participation in local and wider communities.

Maintain a caseload of clients providing ongoing support until cases are handed over to appropriate services. The role involves accompanying clients to appointments, updating casework records daily logs written assessments working closely with other teams across mental health community-based services collaborating on policy development supervision student training monitoring supervising students volunteers implementing existing standards best practice inputting into new ones ensuring adherence work environment maintaining vigilance Health Safety Welfare risks identifying promoting facilitating recovery focusing on housing well-being case managing referrals optimization sustainment independence employ job seekers communication social skills relevant meaningful proactive sought worker contracted requirements preferably prospects foster residents independently navigate external processes client referring practitioner advocate system systematic envisioned real lasting exchange internal assessment accessing advocacy incorporation outcomes sense restrictive phase advance understood taught initiatives consent outreach choices equitable low approach confirm leaving ambitions reception effectively forwarding understand deep prevent session chair priorities funding inspection placement update gathering program rendered transitions cost endured present serve rights violations testimonials violent duties pro attended forefront clearing weeks publication works investigated bodies lend sites negotiated shift greater deduct principles scrutiny follow removing restrictions indirectly restoring.
