About the Role
We are seeking an experienced and skilled CNS to join our team, providing expert nursing services in diabetes management. As a key member of our multidisciplinary team, you will work closely with healthcare professionals to ensure seamless patient care delivery.
Key Responsibilities:
* To provide specialist nursing services in diabetes management, promoting best practice guidelines and evidence-based care delivery
to identify health promotion priorities and support patient self-care; utilize audit results and research findings to inform quality improvement initiatives; contribute to service planning processes through the use of audit data and expertise.
The ideal candidate will possess excellent communication skills, be able to develop strong relationships with colleagues at all levels within our organization.
