Social Worker – CDNT
Medforce are proud to bring to marker this Social Worker role in a thriving CDNT based in South Louth and Bettystown, the aim will be to provide professional social work services to children with disabilities (age range 0-18) and their families, as part of the CDNT, ensuring family‑centred assessment, planning, intervention and advocacy so as to promote child wellbeing, inclusion, and improved quality of life.
Responsibilities of Social Worker – CDNT
* Conduct psychosocial assessments of children with disabilities and their family contexts.
* Develop, implement, monitor and review individualized social work support plans / care plans in conjunction with the child, family, and the multidisciplinary team.
* Provide direct social work interventions: supporting families, counselling, crisis intervention, safeguarding.
* Liaise with other services (health, education, community, voluntary, etc.) to ensure access to resources and supports.
* Support families through transitions (e.g. pre‑school to school, school to adult services).
* Advocate for children’s rights and entitlements: ensuring access to services, supports, educational resources, therapies, etc.
* Ensure protection concerns are identified and appropriately managed (in line with relevant child protection legislation in Ireland).
* Work closely with other CDNT professionals: speech & language therapists, occupational therapists, physiotherapists, psychologists, etc.
* Participate in team meetings, case reviews, joint planning.
* Maintain accurate and up-to-date case records, reports, including assessments, plans, reviews.
* Ensure compliance with relevant legislation (e.g. GDPR, CORU registration standards), HSE policies, child protection policies.
* Participate in supervision, reflective practice, continuing professional education.
* Contribute to CDNT service development, evaluation, improvement initiatives.
* Support parents / caregivers with advice, resources, training if needed.
* Engage with schools, community agencies to promote inclusive practices.
Requirements of Social Worker
* Recognized Social Work qualification.
* Registration with CORU as a Social Worker.
* Experience (post‑qualification) working with children and families; experience in disability services is an advantage.
* Ability to work independently and as part of a team.
* Full driving licence and access to vehicle (for home visits etc.).
