Contract
Permanent Part-time – Flexi part-time contract of 48 hours per month minimum.
Responsibilities
Provide person-centred support to individuals with complex needs.
Plan and deliver support in line with the person's Person Centred Plan (PCP) and care plan.
Work independently, taking initiative in daily support.
Contribute to programme planning and support worker role philosophy.
Qualifications & Experience
Full QQI Level 5 Health Care Qualification (health, community, care sector) or equivalent.
Good communication skills and fluency in English.
Desirable: Full, clean driver's licence (manual).
Skills & Abilities
Ability to work on own initiative.
Awareness of Key Worker / Support Worker role and philosophy.
Focus the needs of the person supported as the key focus in all activities.
Deliver support according to the person's PCP and care plan.
