RGN Required for Social Inclusion Community Healthcare
This is a full-time position working in the Social Inclusion Community Healthcare team at East Clonskeagh Hospital in Dublin 6.
Key Responsibilities:
* Initiate nursing assessments and prepare care plans for all social inclusion groups, including Travellers, Ukrainians, International Protection applicants, homeless persons, and others.
* Prioritize workload and situations as appropriate, working closely with the DON (Deputy Nurse Manager).
* Staff nurses in clinical settings, outreach clinics, and other locations as required.
* Work with the DON to develop new, innovative nursing projects and implement them.
* Promote equality and inclusivity among all social inclusion cohorts, including LGBTQ+, Domestic Sexual Gender Based Violence, Traveller and Roma communities, and others.
* Maintain good working relationships with statutory, professional, and voluntary organizations involved in healthcare.
* Attend clinical/management supervision with the DON.
* Support Primary Health Care Traveller Projects (PHCTP) and social inclusion services.
* Collaborate with PHCTP, IPAS centers, Community Health Networks, community services, and NGOs to ensure integration and optimal healthcare access.
* Work with social care staff regarding patient care.
* Support the delivery of National Screening Initiatives for the Travelling community, as required.
* Work as part of a multidisciplinary team to meet client needs and advocate on their behalf when necessary.
* Respect client dignity and confidentiality.
* Be knowledgeable about Health Service Executive – CHEast policies and procedures and observe them.
* Stay informed about Nursing Policies and Procedures and An Bord Altranais guidelines.
* Manage incidents according to policy and respond effectively in emergency situations.
* Contribute to nursing quality assurance and risk assessments.
* Adhere to standard documentation and evaluation systems.
* Maintain high standards of clinical, professional, and ethical responsibility.
* Promote a safe environment for clients, families, and staff.
* Participate in clinical care audits.
* Participate in groups, committees, and team meetings as required.
* Contribute to the development and implementation of information-sharing protocols, audit systems, referral pathways, individual care, and shared-care arrangements.
* Participate in education/training initiatives for SI services, clients, and Traveller Health Projects.
* Act as a mentor and coach to projects/SI services in implementing healthcare programs.
* Take measures to maintain competence through ongoing education opportunities.
* Participate in in-service training to stay current with issues and maintain competencies.
* Act as a resource person to enable other healthcare workers to achieve positive health outcomes.
Requirements:
* RGNs with experience in social inclusion or work with marginalized groups.
* A self-starter and independent worker.
