Job Description

We are seeking a skilled and dedicated individual to fill the role of Programs Facilitator.


About the Role

The successful candidate will be responsible for promoting and supporting independent living and community integration. This will involve facilitating educational, developmental, recreational, and occupational programs and activities, as well as designing and implementing person-centered plans that cater to the needs and abilities of service users.


Duties and Responsibilities

* To act as a key worker for service users, facilitating the person-centered planning process.
* To identify goals and supports required by individual service users and maintain records as necessary.
* To develop and deliver person-centered programs that meet the needs of service users, as identified through person-centered planning.
* To support individuals and groups in accessing their community, facilitating activities and programs as needed.
* To work independently and liaise with team members to ensure high-quality services for service users.
* To communicate effectively with external agencies and families, maintaining relevant records.
* To adhere to health and safety regulations and promote independence and community integration in all interactions with service users.


Requirements

* A minimum of one year's experience working in a relevant setting, with expertise in facilitating and implementing person-centered plans.
* Experience in designing, developing, and providing programs and activities to meet the needs and abilities of service users.
* Experience supporting adults with intellectual disabilities, autism, mental health difficulties, physical disabilities, and non-verbal communication challenges.
* A specific interest in services and programs catering to people with mental health issues or diagnoses.
* Ability to work flexibly, including evenings when necessary.
* Familiarity with Health, Safety, Hygiene, and Good Housekeeping guidelines and best practices.
* Experience promoting self-advocacy and empowering service users.


Qualifications and Certifications

* Full driving license is essential.
* Minimum QQI Level 5 in Social Care or equivalent qualification.
* A third-level graduate in Health & Social Care or equivalent desirable.
* Additional qualifications in supporting adults with Autism or Challenging Behavior desirable.

Salary: €30,915.76 to €32,518.28 (pro rata)
