OverviewAn exciting opportunity for an experienced RSW to join the Intermediate Care Team within the Integrated Community & Support Services (ICASS) in the Cupar locality. The Intermediate Care Service provides rehabilitation to adult residents in the locality, delivered as in‑patients, out‑patients or in their own homes.ResponsibilitiesWork collaboratively with Occupational Therapy, Physiotherapy and District Nursing Services to deliver therapeutic treatment programmes to service users in ward settings or homes, preventing hospital admission and supporting early discharge from hospital.Provide daily care in the community, available 365 days a year from 08:00 to 22:00.Deliver treatment programmes for a mixed caseload, primarily older adults.Maintain accurate clinical records and participate in regular supervision.Continue professional development through PDP and supported training opportunities.Work flexible hours to cover a 24/7 schedule across 7 days and may be required to work at other community hospital sites.Qualifications• SVQ Level III in Health and Social Care or equivalent. • Experience as a support worker desirable. • Experience working with older people desirable. • Good organisational and communication skills; able to work independently and within a team. • Ability to travel throughout the area essential.BenefitsRegular supervision and PDP support. • Flexible working arrangements. • Family‑friendly policies. • Full support for disabled candidates and those with long‑term or neurodivergent conditions through reasonable adjustments.EEO StatementNHS Scotland is committed to encouraging equality and diversity among its workforce and eliminating unlawful discrimination.
