 
        
        Outreach Social Care Leader
Location
Based in Wellsprings Outreach Service, with community engagement and direct links to the Outreach Coordinator.
Staffing Structure
The outreach service is staffed by a team of Social Care Workers and Leaders under the direction of the Outreach Coordinator, Person in Charge, and Service Manager. The service operates under the auspices of the Sisters of Mercy and is overseen by a Board of Directors.
Details of Service
The outreach component provides tailored support to individuals transitioning to independent living. This includes former residents of Wellsprings, direct referrals, and self-referrals over the age of 18.
General Responsibility
 * Deliver community-based support, empowering clients to achieve independence and well-being.
 * Promote an inclusive, person-centered approach while adhering to professional standards and policies.
 * Act as a liaison between clients and other support services, ensuring holistic care.
Principal Duties
 * Collaborate closely with the Outreach Coordinator to design and implement support plans.
 * Maintain detailed and accurate client records per established procedures.
 * Coordinate and facilitate life-skills training programs tailored to individual needs.
 * Provide professional supervision to Social Care Workers within the outreach service.
 * Build strong working relationships with external agencies, including Tusla, HSE, and voluntary organizations.
 * Attend and contribute to team meetings and training sessions.
 * Support the development and review of outreach programs and services.
 * Assist in the induction and mentoring of new staff.
 * Oversee financial accountability related to outreach service activities.
Keyworker Duties
 * Serve as a primary contact for assigned clients, offering consistent and supportive engagement.
 * Develop, implement, and review individualized support plans in collaboration with clients, families, and professionals.
 * Prepare reports, including case conferences, reviews, and referrals, as required.
 * Ensure clients' concerns and complaints are addressed per organizational procedures.
 * Promote self-reliance and community integration through skill-building and advocacy.
Professional Development
 * Participate in supervision and training sessions regularly.
 * Pursue continuing professional education to stay current with best practices in outreach and social care.
Other Duties
 * Actively contribute to the broader goals of the organization by participating in strategic planning and policy development.
 * Undertake additional responsibilities as assigned by the Manager or Outreach Coordinator.
Hours and Pay
 * Hours: 35 hours per week.
