We are hiring Clinical Nurse Manager- Discharge Co-ordinator into a Hospital in Dublin, Ireland.
Key Responsibilities Assess patients' needs and plan safe, timely transitions from hospital to appropriate post-discharge settings.
Conduct daily inpatient rounds with clinical teams to identify discharges, plan care, and estimate discharge dates.
Coordinate and lead multidisciplinary and complex discharge meetings, ensuring accurate documentation and communication.
Collaborate with patients, families, carers, and healthcare teams to support effective discharge planning from admission to discharge.
Oversee patient flow and length of stay, working with MDTs to streamline discharge processes and expedite care pathways.
Coordinate transfers to external healthcare facilities, including rehabilitation, convalescence, and long-term care.
Build and maintain strong relationships with community services to facilitate smooth and timely patient discharges.
Manage and develop the Patient Lounge function to improve discharge efficiency and patient flow across the hospital.
Ensure compliance with policies, quality standards, and best practices while supporting staff development, training, and performance management.
Monitor performance metrics, contribute to audits, and support continuous service improvement, innovation, and effective resource utilisation.
Requirements Graduate of an accredited Bachelor of Nursing Science degree programme.
Registered, or eligible for registration, in the relevant division of the Register of Nurses maintained by NMBI.
Minimum of 5 years' experience as a Registered Nurse in an acute hospital setting.
At least 2 years' management or leadership experience.
Demonstrated clinical, managerial, and administrative capability to effectively perform the role.
