Programme Facilitator Role
Job Description:
We are seeking an experienced and skilled Programme Facilitator to join our team. This role is responsible for promoting and supporting independent living and community integration among service users.
Key Responsibilities:
1. Develop and Deliver Programmes: Design and implement person-centred programmes that meet the needs of service users, as identified through Person Centred Planning, and maintain accurate records.
2. Key Working: Act as a key worker to service users, facilitating the Person Centred Planning process and providing ongoing support.
3. Goal Setting and Support: Identify goals and required supports for individual service users, maintaining relevant records and ensuring effective implementation.
4. Community Access: Support individuals and groups in accessing their community, facilitating activities and programmes as needed, and undertaking other reasonable duties as directed by the Programmes Supervisor or Community Services Manager.
5. Regulatory Compliance: Adhere to all rules and regulations of the Rehab Care services, ensuring compliance with relevant policies and procedures at all times.
