Senior Diabetes Care Specialist
As a senior specialist in diabetes care, you will be responsible for providing expert nursing services to patients with diabetes who require ongoing support and treatment. Your primary goal will be to enhance the quality of life for these patients by delivering evidence-based care.
* You will conduct comprehensive patient assessments, incorporating physical, psychological, social, and spiritual elements to inform your care planning.
* In collaboration with the multidisciplinary team, you will develop and implement individualized plans of care that prioritize the needs of each patient.
* You will monitor patient outcomes, making adjustments to care plans as necessary to ensure optimal results.
* Effective referral pathways will be established to ensure seamless transitions of care for patients with diabetes.
To achieve this role, you must maintain clinical expertise in diabetes management through ongoing education and training. This includes staying up-to-date with the latest research and best practices in the field.
Key responsibilities will include:
* Providing patients and their families with accurate information and support to empower them in managing their condition.
* Working collaboratively with healthcare professionals to optimize care delivery and promote effective communication.
* Identifying and addressing any knowledge gaps or skill deficits among patients and caregivers.
Eligibility Criteria:
* Current registration with the Nursing and Midwifery Board of Ireland (NMBI).
* A minimum of 5 years post-registration experience, including at least 2 years recent experience in diabetes care.
* Excellent interpersonal and organizational skills, with the ability to communicate effectively both verbally and in writing.
