The role
The post of CNM 2 Discharge Co-ordinator has a pivotal role in the effective co-ordination of patient discharges throughout the hospital and will provide leadership and co-ordination to the wards and specialty teams with regard to patient discharge across Wexford General Hospital, including stepdown and egress beds as required.
The post holder will play a key role in the development of discharge policies and procedures facilitating timely and seamless discharge of patients to follow up care.
The post holder will be responsible for the monitoring of key bottlenecks in the discharge process and facilitate effective discharge planning at ward level.
The CNM2 role is responsible for the management, care and treatment of service users, to ensure that the optimum standard of care is provided within the designated area(s) of responsibility.
The person
Candidates must have at the latest date of application:
* Statutory Registration, Professional Qualifications, Experience, etc
* Eligible applicants will be those who on the closing date for the competition:
* Are registered in the relevant division of the Register of Nurses & Midwives maintained by the Nursing and Midwifery Board of Ireland [NMBI] (Bord Altranais agus Cnáimhseachais na hÉireann) or entitled to be so registered.
And
(ii) Have at least 5 years post registration experience (or an aggregrate of 5 years fulltime post registration experience) of which 2 years must be in the speciality or related area.
And
(iii) Have the clinical, managerial and administrative capacity to properly discharge the functions of the role.
And
(iv) Candidates must demonstrate evidence of continuous professional development.

* Candidates must possess the requisite knowledge and ability including a high standard of suitability and clinical, managerial and administrative capacity to properly discharge the functions of the role.
* Annual registration
* On appointment, practitioners must maintain live annual registration on the relevant division of the Register of Nurses and Midwives maintained by the Nursing and Midwifery Board of Ireland (Bord Altranais agus Cnáimhseachais na hÉireann).

(ii) Confirm annual registration with NMBI to the HSE by way of the annual Patient Safety Assurance Certificate (PSAC).
